GENERAL RELEASE
I agree to allow MYfitness to design a program for me to use as part of my competition preparation, general fitness, and/or health goals. I will not hold MYfitness and/or any related persons or parties personally liable for injuries, illnesses or problems that might occur as a result of the program developed for me. This program does not replace the expert advice or medical treatment of a health care or medical professional. I affirm that I have provided MYfitness with all necessary and pertinent information. I take full responsibility for any and all complications, injuries, illnesses or death that may occur as a result of this program.
PHOTOGRAPHY RELEASE
Whereas for valuable consideration hereby acknowledged as received, the Model granted MYfitness permission to photograph him/her and thereafter to use the photographs in whole or in part without restriction anywhere, in any medium, for any purpose and altered in any way. The Model releases MYfitness from all claims of liability relating to the use of the photographs. This permission and release shall be irrevocable and binding upon the Model's successors, legal representatives and assigns and shall accrue to the benefit of MYfitness successors, legal representatives and assigns.
REFUND POLICY

I understand and agree to the following: The expiration policy requires completion of training and nutrition (or nutrition only) packages within the agreed upon weeks from the date of the contract. All training and nutrition are void after this time period. 4. There is no refund in whole or in part of the initiation fee once paid for any reason, including but not limited to relocation, illness, and unused sessions weekly.
FINAL RELEASE
I understand that my participation and/or involvement in MYfitness carries with it the potential for certain risks, some of which may not be reasonably foreseeable. I further acknowledge that these risks could cause me, or others around me, harm, including, but not limited to, bodily injury, damage to property, emotional distress, or death. I am a willing participant in any and all MYfitness activities, events, programs. By signing this agreement, I agree to release, indemnify, and hold harmless MYfitness, as well as all its employees, agents, representatives, successors, etc. from all losses, claims, theft, demands, liabilities, causes of action, or expenses, known or unknown, arising out of my participation in any and all MYfitness activities, events, programs.
CANCELLATION POLICY

Monthly Subscription: By purchasing a Monthly Subscription (i.e. Membership), you are agreeing to an initial and recurring Monthly Subscription fee based upon the Package you purchase (3, 6, or 12 months) at the then-current Monthly Subscription rate, and you accept responsibility for all recurring charges until the end date of your subscription when you cancel. You may not cancel your monthly subscription until your end date of your subscription.
Automatic Monthly Renewal Terms: Once you subscribe to our Membership, MYfitness will automatically process your monthly subscription fee in the next billing cycle. MYfitness will continue to automatically process your monthly subscription fee each month at the then-current Monthly Subscription rate. When your end date approaches, you must notify MYfitness a minimum of 7 days prior to your end date in order to cancel your subscription. Failure to do so will result in another monthly subscription fee after your Membership has ended and so forth until you notify MYfitness about cancelling your Membership. MYfitness is not responsible for reminding you to cancel your subscription when the time comes.
